Magnus Health Information

Blessed Trinity Catholic High School uses one platform, Magnus Health, for all electronic medical record keeping and GHSA athletic forms. ALL health information must be completed and updated annually via the Magnus system where it is stored electronically. Blessed Trinity follows HIPAA and FERPA guidelines regarding medical records as well the Archdiocese of Atlanta’s policies regarding use and retention of records.

All students must complete their Magnus Health information in order to attend school and participate in athletics. This must be completed for all students by June 1, 2025.

To Access Magnus:

Returning students

All parents will receive an email from service@magnushealthportal.com with instructions to update your student’s forms. All forms must be updated by June 1, 2025.

New students

All parents will receive an email from service@magnushealthportal.com with instructions to create/access your Magnus account. You will have an option to watch a short tutorial video which will help you to navigate the system. An account must be created and all required forms must be submitted for all students by June 1, 2025.

To log in after the initial setting of the password, navigate to www.mymagnus.com or you can utilize the Magnus Mobile V2 app.

Required Forms For Magnus:

The following forms require a physician’s signature and must be uploaded digitally by the parent to Magnus:

Preparticipation Physical Evaluation (please upload all four pages)
- Physical examinations are valid for 12 months from the date performed (exception: any physical examination taken on or after April 1 is valid for the entire following school year)
Certificate of Immunization Form (physician must complete GA DPH Form 3231)
Prescription Medication Form (if needed)
Action Plans for: Allergies, Asthma, Diabetes, Seizures (if needed)
GHSA Forms:
- Concussion Awareness Form
- Sudden Cardiac Arrest Awareness Form

How to update your student's Magnus account

All parents will receive an email from service@magnushealthportal.com.
If you have an existing Magnus account, please update your student’s annual forms.
If you do not have a Magnus account, click Create Password.
Once you create a password, you will be taken directly into your student’s Magnus account. You will have an option to watch a short tutorial video. This will help you to navigate the system and complete the mandatory process for your student(s).

After the initial account set-up, navigate to www.mymagnus.com or use the Magnus Mobile V2 app.

Immunizations

ALL students are required to maintain a current Immunization record; it MUST be on the Georgia Department of Public Health Certificate of Immunization Form 3231 (GA#3231) and be on file with the school by the first day of school or attendance may be suspended. (Handwritten 3231 forms will not be accepted). Please contact the clinic with any questions or concerns.

Effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4), unless their initial dose was administered on or after their sixteenth birthday. If the sixteenth birthday occurs during the 11th grade school year, a 30 day time frame is enforced by state law to become updated with a booster. Georgia DPH Meningitis Vaccine Letter

If your student received the COVID-19 Vaccine, please upload a copy of the vaccine card under the immunization tab. This will not override any other immunizations that are recorded under that tab.

Medication Policies

The Clinic does not dispense Over-the-Counter Medications to Students.
Students are allowed to carry OTC medications in their backpack as long as it is labeled with the student's name and is in its original store purchased travel size container. OTC medication is not to be shared with other students, if so disciplinary actions are possible.

All Prescribed Medications must be dispensed through the Clinic and have a Medication Permit Form on file.
Medication dispensed through the Clinic must be in its original container labeled with the student's name, name of the medication, and administering instructions. The exceptions are EpiPens, rescue inhalers and Insulin, these emergency medications can be carried by students once their Action Plan is completed.

If your child has been prescribed any emergency medication such as an EpiPen (allergies), a rescue inhaler (asthma), insulin (diabetes or hyperglycemia), and/or glucagon (hypoglycemia), parents must provide the following forms and medications to the Clinic with NO exceptions:

Action Plan completed by your physician and parent
Medication consent form signed by your physician and parent
Emergency medication prescribed by your child’s physician in the original pharmacy container with the original pharmacy label

Any prescribed medication found on a student without a Medication Permit Form on file in the Clinic will result in medication being removed from student and possible disciplinary actions.

All student medications must be picked up from the Clinic at the end of the school year. Medications will be discarded if not picked up either by parent or student.

If your child has other health conditions that may need to be addressed, please contact the Clinic.

When Should My Child Stay Home From School?

If a student presents with one or more of the following symptoms, they will be sent home and
may not return to campus until they are fever (100.4) and symptom-free for 24 hours or they receive
clearance to return from a physician.

Loss smell/taste
Shortness of breath/difficulty breathing
Cough
Fever/chills
Nausea/Vomiting
Diarrhea
Sore throat
Congestion/Runny Nose
Muscle/body aches
Headache
Fatigue

Individuals with known/recurring conditions (i.e. seasonal allergies, gastrointestinal issues) may
return to school while symptomatic with a doctor’s note or if it is a recurring issue that is documented in Magnus.

When a student is exhibiting symptoms of an illness, please be aware that the Clinic nurses,
with the direct approval of the principal, have the right to determine when that student is to be
sent home from school.

COVID-19 Protocol (updated August 2024)

Students may return to school when the following criteria have been met:

24 hours fever free without medication
Covid symptoms have significantly improved

Masking is not mandated but STRONGLY encouraged upon student's return to school.
