Application Requirements

Required items for online application to Blessed Trinity

Please note: The state of Georgia and the Archdiocese of Atlanta require up-to-date immunizations for school attendance. The Catholic Church does not recognize an immunization exemption based upon matters of conscience/religious convictions.
 

9th Grade Applicant

* Online Application 
* $100 Application Fee (before February 1st; $150.00 for late applicants after February 1st)
* Applicant Questionnaire
* Birth Certificate
* Psychological Educational Testing (if completed previously)
* IEP (if on file for a public school applicant)
* SSAT taken in 8th grade (important for Admissions consideration AND course placement as part of enrollment)
* Transcript of Grades from 6th, 7th, and the first half of 8th grade
* Standardized Testing Completed in Middle School
* Current Language Arts Teacher Evaluation to be submitted near or after first semester
* Current Mathematics Teacher Evaluation to be submitted near or after first semester
* Current Administrator/Counselor Evaluation to be submitted near or after first semester
* Conduct Record
* Parish/Church Verification Form (to be submitted by pastor or priest at current parish/church)
 

Upper Grades Applicant

* Upper Grades Applicant
* Online Application
* $100 Application Fee(before February 1st; $150.00 for late applicants after February 1st)
* Applicant Questionnaire
* Birth Certificate
* Psychological Educational Testing (if completed previously)
* IEP (if on file for a public school applicant)
* PSAT or Pre-ACT from current school year (if not taken, then applicant will need to take SSAT)
* Transcript of Grades from current school year (through first semester) and two prior school years
* Standardized Testing completed in current school year and two prior school years
* Current Language Arts Teacher Evaluation to be submitted near or after first semester
* Current Mathematics Teacher Evaluation to be submitted near or after first semester
* Current Administrator/Counselor Evaluation to be submitted near or after first semester
* Conduct Record
* Parish/Church Verification Form (to be submitted by pastor or priest at current parish/church)
